We have a vacancy for a Clinical Nurse Specialist to join the Community Team, providing and coordinating 24/7 care for those living with life limiting illness.
Advise patients in relation to their illness and their families and carers on their specific needs and to enable expression of preferences and wishes in relation to their care.
Provide, implement and evaluate individualised end of life care, recording in the care plan and actively participate in multi professional meetings, acting as patient advocate and representing nursing aspects of care.
To triage referrals and urgent calls during working hours, providing a rapid response assessment where required or signposting to the Hospice at Home Department.
Assess and manage symptom control with support from the Multi-Disciplinary Team, pre-empting preventable, physical symptoms and reducing psychological distress.
You will be required to participate in the Community Team weekend on call rota.
To apply you will need to be a Level One registered nurse with a minimum of three years post registration experience, preferably with experience in the community. Palliative care/oncology experience and experience of working with imminently dying patients is essential. You should be able to work autonomously and manage a small caseload as a lone worker. You will also need excellent communication and interpersonal skills and be able to build effective working relationships. Car driver essential and access to your own vehicle.
