This is a new development opportunity for an experienced Staff Nurse or Senior Staff Nurse to join the Community Team, following a 1-2 year development plan.
Advising patients in relation to their illness and their families and carers on their specific needs and to enable expression of preferences and wishes in relation to their care.
Providing, implementing and evaluating individualised end of life care, recording in the care plan and actively participating in multi professional meetings, acting as patient advocate and representing nursing aspects of care.
Triaging referrals and urgent calls during working hours, providing a rapid response assessment where required or signposting to the Hospice at Home Department.
Assessing and managing symptom control with support from the Multi-Disciplinary Team, pre-empting preventable, physical symptoms and reducing psychological distress.
There will also be a requirement to participate in the Community Team Weekend On Call Rota.
To apply you will need to be a Level One Registered Nurse with a minimum of three years post registration experience. You will be capable of working autonomously and managing a caseload following your development in this role. You will need excellent communication and interpersonal skills and be able to build effective working relationships.
Car driver essential and access to your own vehicle.
Please note that the position may be based in Welwyn Garden City or Standon in Hertfordshire and therefore some flexibility is required.
