Our Hospice @ Home team has a vacancy for a Palliative Care Support Worker to join the team to provide holistic care to patients, families and carers, offering practical and emotional support in the patient’s home at the end of life. The role will provide individualised, high quality care to end of life patients including assisting with nutritional needs, helping to move and mobilise patients, providing personal care, respecting privacy and dignity at all times. You will also help to enable and support family and friends to continue to care for the patient and ensure effective communication with other members of the primary care team, escalating concerns where appropriate.
To be considered for this position you will need to have previous experience working as a Health Care Assistant and providing personal care, preferably in the Community setting and with terminally ill patients. You will need to hold an NVQ Level 2 in health and social care and be willing to train towards Level 3. You will need to be able to work independently in the delivery of patient care and be able to demonstrate an awareness of the physical and emotional needs of patients.
The Hospice @ Home service operates from 9am-9pm seven days per week and shifts will be six hour shifts and will include weekends and bank holidays on a rota basis. Please note that the post is based at Head Office in Welwyn Garden City but is lone working role and you will need to have a full driving licence and use of your own car.
