Providing a wide range of care and support to people with life limiting illnesses and their families, we have cared for 1,555 patients across all of our services during the last year alone.

We have a vacancy for a Health Care Assistants to join our Hospice at Home Team to provide holistic care to patients, families and carers and to offer practical and emotional support to patients who are nearing the end of life. Working in the community you will visit patients in their own homes and provide individualised, high quality care including assisting with nutritional needs, helping to move and mobilise patients and providing personal care. You will also help to enable and support family and friends to continue to care for the patient and ensure effective communication with other members of the primary care team.
To be considered for this position you will need to have previous experience working as a Health Care Assistant and providing personal care, preferably in the Community setting and preferably with experience of providing care for terminally ill patients. Ideally you will need to hold a Level 2 Health and Social Care qualification but not essential. You will also need to be able to work independently and as part of a team in the delivery of patient care and be able to demonstrate an awareness of the physical and emotional needs of patients.
You will need to have a full driving licence and use of your own car.
The Hospice @ Home service operates from 8.30am to 10pm seven days per week and shifts will be 7.5 hour shifts from 8.30am to 4.30pm or 2pm to 10pm and will include weekends and bank holidays on a rota basis. Please note that the post is based at our Head Office in Welwyn Garden City but you will need to travel to see patients in their own homes in the community either in pairs or as a lone worker dependent on patient requirements.
