How to make a referral
All health and social care professionals can make a referral for patients with life limiting illnesses requiring specialist palliative care, to achieve control of symptoms, social, psychological, spiritual and support for the patient and family. This can be for short interventions where discharge is possible, for end of life care or for support during palliative treatment.
How are referrals prioritised?
Referrals are prioritised according to the complexity of problems presented and the response time requested on the form.
How do I refer to Isabel Hospice?
Referrals for Isabel Hospice can be made by completing our Referral form.

What is our catchment area?
Our service covers General Practices which are located in the Boroughs of Broxbourne, Welwyn Hatfield and East Herts District. Patients from other areas will be considered on an individual basis.
Discharge from our care
The Isabel Hospice In-Patient Unit is not a long stay facility and patients whose symptoms have stabilised will be discharged. Patients being cared for in the community who are clinically stable and no longer require specialist palliative care input will be discharged from the service but in all cases patients can be re-referred if the need arises.
