ABOUT THE COURSE
Isabel Hospice and Garden House Hospice Care are delighted to be working alongside the palliative educationalists at Lister Hospital and HCT to support our nurse specialist teams in the work we do. This half day is an opportunity for nurse specialists and advanced practitioners to meet each other from different areas. There will be educational topics alongside an awareness about organisations who are there to support the work we do. There will be an opportunity to learn from each other and we hope the Forum will evolve to meet your highlighted training needs.
This is a collaborative venture and so we would love to hear from you is you would like to support or have wish for us to explore an area of interest at these forums. Please contact nicky.wood@isabelhospice.co.uk to discuss further.

Introductions
Discharge Planning/Discussions/Fast Track and what is available to patients and their families –Â session to be led by Integrated Discharge Team (IDT) from the acute trust
Referrals to District Nursing service and what can be offered – to be led by District Nurse Lead
Dispelling the myths around hospices – services available for outpatients and referrals into the hospice for inpatient stays
WHO SHOULD ATTEND?
This session is for any clinical nurse specialists or advanced practitioners who wish to create collaborative links and learn from each other.
